All participants of the LiL Kids Lax program must agree to the waiver form below prior to the first session of our program. 

 

 

RELEASE AND WAIVER

 

 [___________________________], the child's parent and/or legal guardian, understand the nature of LiL Kids Lax LLC activities and the child's experience and capabilities and believe the child to be qualified, in good health, and in proper physical condition to participate in such activity.

 

I hereby release, discharge, and covenant not to sue LKL LLC, their respective administrators, coaches, directors, agents, officers, members, volunteers, employees, other participants, any sponsors, advertisers, (each considered one of the "Releasees" herein) from all liability, claims, demands, losses, or damages on my and my child's account caused or alleged to be caused in whole or in part by the negligence of the "Releasees" or otherwise, including and I further agree that if, despite this release and waiver of liability, assumption of risk and indemnity agreement, I, or my child, or anyone on the child's behalf, makes a claim against any of the "Releasees", I will indemnify, save, and hold harmless each of the "Releasees" from any litigation expenses, attorney fees, loss, liability, damage, or cost which may incur as a result of such a claim.

 

I understand and agree that LKL LLC.  has the right to terminate the enrollment of any child from LiL Kids Lax if it, in its sole discretion, determines that the continued attendance of the child in the program is not in the best interest of the child nor the program.

 

I agree to supervise or designate a responsible adult to supervise my child prior to and immediately after his/her LiL Kids Lax activities.

 

I, the Adult Participant [or Minor Participant and Parent(s) or Guardian(s)], (hereafter referred to as Participant/Parent)understand that all activities of LKL LLC include inherent risks that cannot be totally eliminated regardless of the care taken by LKL LLC. Participant/Parent 1) know, 2) understand, and 3) appreciate the types of injuries inherent in LKL LLC activities. Participant/Parent hereby assert that Participant/Parent knowingly assume all inherent risks of the activity.

PRIVACY POLICY

 

It is the policy of LKL LLC. to never release, sell, or otherwise divulge information submitted electronically or in writing to any third party without the expressed consent or knowledge of its customers. We employ SSL Security Encryption whenever possible to ensure that all information sent electronically is as difficult as possible for any third party to decipher.

 

I understand that a photo or video clip may be taken of the class, including a photo or video clip of my child. I hereby authorize LKL LLC. to take such photos and or video clips for promotional activities. I consent to LKL LLC use of my child's photograph for advertising purpose, and waive all rights to compensation and other rights which may arise as a result (including any rights under N.Y. Civil Rights Law 50).
